He is most likely infected with the tinea cruris fungus, called Dobie’s Itch, or more commonly known as "jock itch" . The fungus thrives in dark, sweaty and airless creases of skin. He can try using off the shelf anti-fungal creams, powder or spray used for Athlete’s Foot. He should keep his genital area clean and dry and wear loose fitting clothing below the waist, or none at all when possible.
If proprietary treatments don’t work, he should get prescribed medication from his doctor, which is more efective and quicker acting.
Get him to read all the info in the link below.
Jock itch is very common, but alternatively, there is a slim chance the fungus could be candida (thrush), but the treatment and medicines will be similar.

If it is a fungal infection, it can persist for months or longer. It usually starts in the gut (intestines) and spreads to warm moist areas of the body. He should see his doctor for a diagnosis so that he knows exactly what he is dealing with. The doctor can advise him how it can be treated. The website below is very informative. It leans toward a natural treatment that will likely work. It should be considered along with the doctor’s advice.
